Intensive Online Pain Management Programme

During the COVID response and in place of seeing our patients face to face we developed an intensive online pain management programme (vPMP) facilitated by a Clinical Psychologist and Specialist Pain Physiotherapist and delivered via videoconferencing software.

The structure of the vPMP comprises:

  • A Pre-vPMP Meet and Greet (1hr)
  • 12 x 90min Sessions: 3 x week for 4 consecutive weeks
  • Capacity for 8pts
  • Comprehensive Workbook and YouTube Channel (Chelsea Pain Clinic)
  • 3 and 9 month virtual group follow-up
  • Focus Group

The content of the vPMP comprises:

  • Grounding Exercise/Mindfulness
  • Consolidation of Learning from previous session
  • Physical Exercise/Movement
  • Relaxation Strategies
  • Sleep Intervention
  • Modules/Topics x 12
  • Committed actions for behavioural change

PROMS being collected to measure changes in:

  • Confidence to perform activity with pain (PSEQ);
  • Specific Patient Function (PSFS);
  • Mood (PHQ);
  • Quality of Life (EQ5D)
  • Work status

What is needed to sustain the change?

Infrastructural support from Trust:

  • Access to appropriate hardware, software, confidential spaces 
  • Training for staff to support digital literacy
  • Integration with hospital systems for appointments and administrative support for email invites, for example

Hi John- thank you for this really helpful post, detailed overview of your programme content and enablers- any top tips/lessons learned for other PMP services moving to this delivery strategy and any participant feedback/considerations ? Thank you

Thanks for taking the time Carol. I'd say top tips include: having appropriate delivery vehicle (we're using Zoom) and supportive infrastructure (integration with hospital systems (we're not there yet); content-wise making it an experiential process for the pts i.e. less didactic lecturing and more co-construction/participation (breakout rooms, in-session movement, reflections)
